CARE HARMONY LENDS A HAND

At Virginia Urology, we know managing your health can be overwhelming – but there are ways to make things easier. We offer eligible patients a Medicare-sponsored program that allows us to proactively help manage your health, even when you’re not visiting the office.

Our new CareHarmony Care Coordination Service includes monthly telephone assistance between regular appointments. Each month, a Care Coordinator will call you to:

  • Assist with scheduling appointments, lab tests, or other tests
  • Explain how and when to take medications
  • Coordinate home health or medical equipment needs
  • Connect you with health education resources, services and programs
  • Identify available community resources
  • Coordinate follow-care after leaving hospital
  • Access other support services you need to help you stay well

As part of this service, you are granted access to a 24/7/365 Care Coordinator Line to help answer any questions you might have or to provide additional assistance.

This service is covered by Medicare and is subject to your annual deductible and coinsurance (usually 20%). If you have a secondary or supplemental insurance plan, your coinsurance may be covered. It is best to review exactly what your insurance plan covers.

A CareHarmony Care Coordinator will be calling you soon to explain the service and answer questions. You can enroll in this service or decline it at any time. If you have questions or would like to enroll now, please call 804-799-2257 to speak with a Care Coordinator.

Once you are seen in our office, you will be sent an email to sign up for the new Athenahealth portal. Please follow directions in the email to activate your new account. Once you have registered for the new portal you may sign into the new portal.

Information in the previous portal will not transfer to the new Athenahealth portal. We recommend that you download any information from the current portal before the end of year if you want to retain copies for your records.
